Where a driver is applying to renew their driving licence or learner permit or to exchange a driving licence from another European Union member state, they are not required to submit an eyesight report form unless they wish to remove a 01 notation (the requirement to wear corrective lenses when driving), for example after corrective eye surgery. An eyesight report may be required when exchanging a licence or if you have previously worn glasses, for further information contact NDLS. We can print the necessary forms at the surgery for you or they can be obtained from your local NDLS centre or downloaded from their website If you are downloading the medical report form you must print it back to back. Please note that it is necessary to present this report at the NDLS centre with your application within one month of completion by the doctor. For further information An eyesight report is also required for a first-time application for a learner permit* which the doctor can also complete for you during your appointment. ![]() Medical reports are required if you are applying for a learner permit or Driving Licence in any of the categories C1, C, D1, D, CE1, CE, DE1 or DE as well as for those over 70 years of age or suffering from certain disabilities or medical conditions.
